- What is your birth date?
- What was your name at birth and have you ever changed your name?
- Where were you born?
- What is your Social Security Number or Social Insurance Number?
- Where do you live?
- What are your phone numbers?
- What is your passport number?
- Where was it issued?
- What evidence do you have that this is all true?

So what happens if a person does have two SIN numbers? And is using both?

As the SIN is used as a tax-payer identifier a person can not have two active SIN numbers. If the person appears to use two SIN’s, then something improper is happening.
